There was a discussion in court if Smallwood had the intention to inflict his disease on the women that he attempted to murder and raped them. I believe he intentionally did want to inflict the disease on the women because he was clearly aware of the fact that he has an illness that is fatal. yet he made the women go through this ordeal. He even confessed that he wanted to kill them therefore he knew that even if they survive, they will be killed sooner or later by the disease. This should be proof enough for the court that he had the intent to kill and a very strong one, this is complete sadistic behaviour.
